Did you receive a letter from Molina Healthcare regarding your Medical Renewal?

If you renewed your Molina Healthcare plan by now OR have signed up for a NEW medical plan, please disregard the letter you received from Molina.

What if I didn’t renew my health care plan?

You may still be able to renew your medical coverage. To renew your health plan:

1. Complete the Eligibility Review Form you received from DSHS.
2. Mail the Eligibility Review Form to your local Community Service Office (CSO).

What if I no longer have, never received, or have questions about the Eligibility Review Form?

Contact your local Community Service Office.

To complete a NEW application:

• Use the Benefit Finder to screen your family for health insurance online.

• Or call the Healthy Kids Now! hotline at 1-877-543-7669. They can send you a new application or help you find your local Community Service Office.
